This authorization is given to the clinic and/or office and/or the doctor Dr José Cury, as well as his assistant(s) and/or other professional(s) selected by him/her to intervene in the procedure(s) and in accordance with his/her professional judgment.
At any time, you, the holder, may:

– request the update or correction of your data;
